How do I enable 'Username' field in Universal signup page?

When customizing the signup prompt text, it is possible to see a Username field at the page preview and it is even possible to customize this field placeholder and error messages.

However, my app signup page only shows the Email address and Password fields. How can I configure my Universal signup page to show the Username field too?
