I need First name and Last name input fields before the email input field how to do that.

You can customize the fields through go to universal login → Advanced option and login and enable customize login page. you must select the application top right to specify in which application you want to make changes.Please refer the following link

I add the input fields through the lock then how to arrange the input fields in the custom html page. how do I take the input fields from the lock and arrange the custom fields. your documentation is not that much useful there is not clear explanation and example
