Failed Login, Missing required parameter: response_type

At least one of our users is experiencing failed logins with the following “Description”: Missing required parameter: response_type. I’m unable to tell who the user(s) is/are (the error message contains no identifying information), so I can’t reach out to them to ask them about it. This is extremely problematic. I’ve seen this question asked before, but there are no good answers from Okta anywhere in the forum. I’m posting again to ask Okta to please address this issue. I can’t ask the user to collect an HAR if I don’t know who they are, and I’m not able to reproduce the error myself as it is completely unclear what is causing it. Please help!
